Abstract

Chronic kidney disease is a clinical condition of decreased kidney function
caused by various causes and pathophysiological processes. This chronic kidney
disease can cause irreversible kidney function decline so it requires special treatment
including hemodialysis or even kidney transplantation. The many costs that must be
borne by patients as long as patients undergo hemodialysis which is not small in
number makes the burden of dependents increase. This research aims to determine the
cost of illness and the level of quality of life of patients with chronic kidney failure
with hemodialysis measures that include direct medical costs, direct nonmedical
costs, and indirect costs, as well as comparison of real costs with INA-CBG's rates
based on the Republic of Indonesia Health Regulation Number 64 of 2016 on chronic
kidney failure patients with hemodialysis measures at PKU Muhammadiyah
Gamping Hospital.
This type of research is observational with cross sectional research design
according to the societal perspective with the cost of illness method. Data collection
was carried out by interviewing patients and companion patients using a
questionnaire and also using medical record data and data on the cost of treating
chronic kidney failure patients with outpatient hemodialysis measures. Data analysis
used descriptive statistical analysis methods with 97 patients. Comparison of real
costs with INA-CBG rates based on using independent t test statistical tests for
normal data and Mann-Whitney statistical tests for abnormal data.
The total cost of illness for patients with chronic kidney failure with
hemodialysis measures which includes direct medical costs, direct nonmedical costs,
and indirect costs is Rp.611,103,400 with an average of Rp203,701,133 ±
330,640,939. The biggest component of direct medical cost is the cost of
hemodialysis. For comparison with INA-CBG rates, a p value of 0,000 is obtained. It
can be concluded that between the real costs and INA-CBG's tariffs have a significant
difference with INA-CBG's rates lower than the average real costs. While the level of
quality of life in patients seen with an average utility value of 0.83 and the average
value of EQ-VAS utility of 75.92 which shows the patient in good condition
