Abstract

Albumin is one of the main proteins synthesized by the liver and the largest
plasma protein is around 55% - 65% of the total plasma protein. Albumin synthesis
decreases in various diseases, especially liver disease. This condition is often known
as hypoalbuminemia (serum albumin <3.5 g / dL) which can endanger the lives of
patients. Albumin infusion is a difficult preparation in the production process and has
limited market sales and high prices. This study aims to look at the most common
comorbidities suffered by albumin therapy patients and the most expensive cost, the
average direct medical costs and the percentage of albumin costs to the cost of the drug.
The method used in this research was non-experimental observational
descriptive and cross-sectional design in the inpatients for the period June 2018 - June
2019. Data collection was carried out retrospectively in August - November 2019 using
the Medical Records and Inpatient Accounts of the patients. The population in this
study was 68 patients and a total sampling technique was performed. Data analysis in
this study used descriptive analysis based on the main diagnosis.
The results of this study show the diagnosis of patients receiving albumin
therapy (at least 2 patients) are Congestive Heart Failure, Diabetes Mellitus,
Abdominal Pain, Ascites, Chronic Kidney Disease, Dyspepsia, Anemia, Miliary
Tuberculosis, Hypertension, Sepsis and Hypovolemic Shock. The most direct picture
of the medical costs of patients receiving albumin in the diagnosis of Abdominal Pain
with an average of Rp. 26,423,934, - covering the cost of room accommodation,
medical expenses, hospital facility fees, laboratory fees, visit fees, doctor's services and
consultations, and other health costs. The average direct medical cost for all albumin
therapy patients is Rp. 13,691,239. The highest percentage of albumin costs for drug
costs is at Anemia diagnosis, which is 45.87%, which is almost half the cost of
medicines. The expensive cost of albumin therapy makes cost efficiency must be done
by paying attention to the effectiveness of therapy.
