Abstract
Background: Diarrhea remains one of the five leading causes of children's deaths in Indonesia with high morbidity and mortality rate. In most cases, death due to diarrhea is caused by dehydration. Clinical diagnosis based on history taking and physical examination, therefore experienced doctor has critical role in detecting dehydration sign.
Objective: To evaluate physical examination in children with diarrhea and to determine the influence of doctor 's job tenure with completeness of physical examination. •
Methods: This was an observational analytic study with cross sectional design. The subject was emergency department (ED) doctors with job tenure less and more than 1 year at Yogyakarta City Hospital. The study was conducted on medical record of diarrhea pediatric patients from February until November 2017, assessed using a checklist. Data was analyzed using Chi-Square, Fisher Exact, and Mann-Whitney test.
Results: 10 ED doctors and 85 diarrhea medical records were enrolled. No doctors conduct a complete physical examination. Nutritional status and impression of illness is only carried out in 15% cases. Body weight is carried out in 75% cases, whereas it has very important role in the treatment of diarrhea. There is no influence of job tenure with assessment of body weight, consciousness, impression of illness, nutrition status, temperature, radial pulse, respiration, hyperperistaltic, sunken eyes, skin turgor, CRT, extremities, and leg pulse. Doctors with job tenure less than 1 year have more completeness in doing physical examination than doctor with job tenure more than 1 year, i.e. 76% compared to 70,69% (p=0,06 5).
Conclusion: Job tenure has not significantly influence to the completeness of physical examination on patient with diarrhea, doctor with job tenure less than I year have more complete in doing physical examination.
