Abstract
Diabetes mellitus is a chronic metabolic disease that can be found in many countries. The prevalence of diabetes mellitus is high on elderly people. Elderlies are prone to receive multiple therapies or polypharmacy which has the potential of PIMs (Potentially Inappropriate Medications) thus possess the risk of therapy failure. Therefore, special attention and treatment are required for elderly patients with diabetes mellitus type 2. The purpose of this research was to determine the description of the medication profile and the suitability of medication pattern in elderly patients with diabetes mellitus type 2 in the Outpatient Installation of PKU Muhammadiyah Yogyakarta Hospital. This research was a descriptive study which collected the data retrospectively. This study used total sampling method on 246 medical records of elderly patients with diabetes mellitus type 2 in the Outpatient Installation of PKU Muhammadiyah Yogyakarta Hospital in 2018-2019. The medical records were analyzed to know the cases of Potentially Inappropriate Medications (PIMs) using the guideline of American Geriatrics Society Beers Criteria 2019. The result of the research showed that patient medication profile was divided into two: antidiabetic and non-antidiabetic medications. The most common antidiabetic medication was found on oral antidiabetic, which was metformin at 33,09%, and the highest insulin use was insulin aspart at 20,00%. The highest non-antidiabetic medication was found on antihypertension drugs at 34,95%. The study result on medication pattern based on Beers Criteria 2019 showed that the percentage of Potentially-Inappropriate Medications (PIMs) on elderly patients was identified to be at 45%.
