MODEL PROMOSI KESEHATAN BERBASIS KELUARGA UNTUK MENINGKATKAN DUKUNGAN KELUARGA PADA PASIEN PENYAKIT KRONIS DI RS

Background: The increasing number of chronic diseases has an impact on increasing the cost of health services. Implementation of family-based health promotion hospital programs can prevent the increasing costs of health services. The purpose of this study was to create a family-based health promotion hospital model to increase family support for chronic disease patients. Method: This qualitative research was conducted through focused group discussion (FGD) and in-depth interview with patients, patients’ families, hospital staffs, HPH team, director, internist, one in charge of IT, and Muhammadiyah leader. The data were collected from July-September 2021. The analysis used is thematic analysis. The analysis was carried out from data collection, data reduction, data presentation, and drawing conclusions.Validity test is done by conducting audit trail, source triangulation and method triangulation. Result: From the transcript of four FGDs and six interviews, a family-based health promotion model was obtained. The family-based health promotion model to increase family support for chronic disease patients in hospitals is formed from eight (8) elements, namely: 1) Health literacy, 2) The preferred method is peer-led discussion, group discussion, 3) Social media using WhatsApp Groups, 4) Forming groups involving families, 5) Involving community leaders, 6) Relaxing and entertaining activities, 7) Effective communication, and 8) Nurse engagement. Conclusion: The main key to the success of the family-based health promotion hospital model is the peer-led discussion method. This method is expected to increase family health literacy so that it can increase family support in treating chronic diseases
